As the Care Coordinator you will be reporting into the Registered Care Manager and support where needed to ensure compliance with the CQC and the Local Authority Quality Assurance Team are adhered to. This is an office-based, care operations role with great responsibility towards staff and clients alike. You will coordinate and manage the care assistants’ team whilst providing them with the necessary leadership and motivation to deliver their best possible work to clients. You will be ensuring all care visits, working pattern and work rotas are coordinated in the most efficient manner to cater to the needs of the customer, company and care assistants.
Main duties and responsibilities:
- Liaising with all care staff, existing and prospective clients, and various health professionals, to ensure the smooth running of their services and provide Safeguarding whenever necessary.
- Expected to participate in the ‘Out of Hours on Call’ rota duty.
- Assist with identification of further business opportunities outside of the current client base.
- Regularly visit clients and write Care Plans or make reviews to existing Care Plans, examine the clients’ electronic MAR charts and daily log notes.
- Responsible for the organisation and participation of all team meetings, staff supervisions and spot-checks on a bi-monthly basis.
The successful Care Coordinator will need to demonstrate a good level of experience within the domiciliary care sector (ideally a minimum of 1 years’ experience). You will be studying or wanting to study towards Health and Social Care Level 3 Diploma. You will have proficient IT Skills including rota planning systems, Outlook, Word, Excel. You will have a great attitude with an adaptable and friendly personality and be a quick learner. You will have the ability to problem solve and seek the best possible solutions even when working under stress; ensuring you always remain polite and respectful of all colleagues and clients. You will need to hold a full driving licence- this is required for the role.
This Care Coordinator is a full-time position offering a salary of £21,000 per annum + £75 per week paid On Call. Any additional work over weekends or evenings that will also be paid extra.
